Dropped scaffold tube and wedge

A 2’ scaffold tube with a right angle attached fell 25’ grazing an employee’s left shoulder and knee before landing on the ground in front of them.

Findings:

• It was determined the band and wedge of the right angle had come loose over time due to vibration.

• Wedge on right angle holding the scaffold tube was driven the wrong direction and was butted up against the handrail.
