Incorrectly Supplied Air Horn – contained propane/butane propellant

While moving breathing apparatus rescue equipment in the habitat for the port slops tank, it was identified that the Air-Horns in use were not as per equipment normally used.

Upon further investigation, it was noted that the cans contain a propane/butane mix.

Two cans were removed from the habitat, and a further 4 cans were found in the vendor's work container.

Findings / follow-up actions:

  • The vendor that supplied the equipment was unaware of the potential hazards with the items
  • The vendor checked their ERP system and confirmed they did not have this type of air-horn on any other offshore location :
  • All air horns on site were checked to ensure they are safe to use and contain no flammable gasses
