This animated film and presentation aims to:- help people be better equipped to recognize when a barrier on the right-hand side of the bow-tie has been impaired and know what action to take to report it- assure the workforce at muster that there are plans and processes in place and engineered equipment to keep them safe
This narrated safety moment describes a major incident which occurred in the brewing industry. However, the root cause of failure to put appropriate isolation controls in place is applicable to any high hazard industry. A large uncontrolled release of ammonia during the removal of a compressor from a refrigeration system at a brewery killed one contractor, seriously injured another with a further 20 people required medical treatment.
A new tool to help Safety Reps inspect Major Accident Hazard sites has been added to the Safety Rep Essentials toolkit.
