It is important for every activity that requires breaking containment, that an appropriate isolation plan for the specific activity is used and followed. Isolation plans should match the particular task and be based on up-to-date process safety information (e.g., P&IDs). It is good practice to discuss isolation tasks and to raise concerns before the task begins.

A blind flange was being removed to replace corroded bolts. The valve was connected to a system which had been previously depressurised.
During routine operation, an audible gas release was detected by a platform operator. The release was investigated and the source was identified.
A 2” closed drain line was subject to bristle blasting in order to allow further inspection and fabric maintenance.
